Subscription

Subscription cost is $100.00 flat fee for two months for all patients. This can be paid at one time or in smaller increments bimonthly at no extra cost.

Late Fee: There are no late fees for the first two times a deadline is missed. A cost of $15.00 will be incurred as a late fee on the third missed payment date.
